New Client Forms

Pre-EA Forms

For families with a scheduled Educational Assessment (EA), these forms help us learn more about your student before our conversation. Please complete at least 48-72 hours before your scheduled EA.

There are three forms included in the Pre-EA form links below.

(1) The Pre-EA form: Elementary or Middle school students should complete the form with you. High school or college students should complete their questionnaire independently. The more detail and practice test documents provided, the better.

(2) For high-school students, please complete the College and Major Interest Form as a family, and upload it to the Pre-EA form before submitting.

(3) The Client Info Form – please see the description below. We ask that you provide your child’s contact information so we can add them to their WhatsApp support group.

Client Information Form

The Client Info Form gathers important contact information for students and families.

If your child is in 8th grade or older, we strongly encourage you to provide your child’s phone number and email. At Firat, we believe in helping students take ownership of their educational journey—and that begins with giving them a voice in their own scheduling, communication, and accountability.

Once you are onboarded, we will create a private WhatsApp group for your family and the Firat team. This gives everyone one simple, secure place for updates, scheduling, quick check-ins, and student accountability.

Availability Form

The Availability Form lets us know when your student is available for sessions. Please include all possible available days/times. You’ll also have a space to indicate preferences. We will always prioritize preferred times if possible. For example: Available: Monday–Thursday 4:30–9:30 PM, Sundays 10 AM–5 PM Preferred: Monday/Wednesday 4:30–6 PM, Tuesday/Thursday 5–7 PM, Sunday 12–5 PM You’ll also be able to address your preferred session length (1 hr or 1.5 hrs).

Zoom vs In-Person

All of our tutors work via Zoom and are trained in best practices for virtual engagement. For families local to Houston, we offer in-person sessions at our office located in Spring Branch at 7521 Westview Dr. For families with packed schedules or limited availability, we strongly recommend Zoom or hybrid (both Zoom and in-person) session preferences for scheduling flexibility.
